Background Screening Disclosure
I hereby authorize The Koleman Group LLC and its designated agents and representatives to conduct a comprehensive review of my background, through a consumer report and or/an investigative consumer report to be generated for employment(or for any other service that requires a background screening), promotion, reassignment, or retention as an employee. I understand the scope of the consumer report/investigative consumer report may include, but not limited to, the following areas: names and dates of previous/current employment, work experience, worker's compensation claims, criminal history records(from local, state, federal, international and other law enforcement agencies' records), sexual offender's list, wants and warrants records, motor vehicle reports, military records, educational verification, license verification, credit history, civil cases, social media, terrorist list, OIG/GSA, OFAC/ any sanction list, Fbi and state fingerprinting and drug testing. Upon request The Koleman Group LLC 15 N 1st St Ste D, Belleville, IL 62220 will supply a copy of the completed consumer report along with a copy of an individual's rights under Fair Credit Reporting Act. Reports are not to be used for criminal activity. The Koleman Group LLC is not responsible for the use of any reports after purchase.
The Koleman Group LLC Use of Records
The Koleman Group LLC will destroy all records of guest(consumers without an account) consumers within 5 days of purchase. However, records of consumers who have registered accounts will have all records viewable to them until the consumer chooses to delete them or deactivate said account. To reduce risk of fraud and/or any other inappropriate behavior with consumer reports. The consumer will have to pay for additional reports and new reports. The Koleman Group LLC is not responsible for the use of any reports after purchase. These reports are not to be tampered with under penalty of law.
Information Collection And Use

Refund Policy
As a consumer report being an intangible service. There are no refunds for any services provided by The Koleman Group LLC. Unless The Koleman Group LLC makes the error/errors for example misspelled words on its end. The refund will only be eligible for 30days after purchase of faulty consumer report. The Koleman Group LLC is not liable for any misinformation/false information, which includes: misspelled names, dates, addresses, ssn numbers, false information. This will not be refunded.
Termination of Service
In violation of any of these terms and conditions, The Koleman Group LLC can take legal action and/or terminate ties with customers, employees, agents, third parties, affiliates and so on. The customer may terminate their services at any time. The Koleman Group LLC may terminate the services provided to the customers at any time for any, unlawful, or unethical uses.
Copyright and Trademarks
All copyrights, trademarks, trade dress, other intellectual property and materials, including page headers, images, text, illustrations, formats, logos, designs, icons, photographs, programs, downloads, video clips and written and other materials (the "IP") are owned, controlled or licensed by The Koleman Group LLC, one of its affiliates or by third parties who have licensed their materials to The Koleman Group LLC and are protected by U.S. and international trademark and copyright laws.
